Your care after treatment or an appointment

We’re with you for every moment, even after your appointment

After your outpatient appointment or treatment, we may want to see you again to check how you’re doing. This is called a follow-up.

Follow-ups might happen before you leave clinic, or later after tests. They can take place in person, by phone, or by video call. We will explain what’s right for you.

Make an appointment only when you need one

In some cases, you don’t need regular fixed appointments. Instead, you can book one yourself if you have a problem. This is called Patient Initiated Follow-Up (PIFU).

  • You only come in when you need care.
  • It saves you time, money and unnecessary trips to hospital.
  • You’ll get clear instructions on when and how to contact us.

Your GP will also be told if you are on PIFU. At the end of your agreed PIFU period, we will decide whether to discharge you to your GP’s care.

Tailored support to keep you safe and well

If you’ve had cancer treatment, you may be offered a personalised type of follow-up. This is sometimes called Personalised Stratified Follow-Up (PSFU). It is designed to give you the right support while avoiding unnecessary hospital visits.

This may include:

  • Regular scans or tests (with quicker access to results).
  • Rapid access back to your specialist team if you’re worried.
  • Advice on symptoms to look out for.
  • A care and support plan to help you stay well long-term.
  • Information on self-management and local support groups.

Your clinical nurse specialist will explain your pathway and guide you through each step.
