Contact us

If you’ve received a text message asking whether you still need treatment for your health condition, don’t worry - it’s not a scam.
We are contacting patients on our waiting lists to ensure they receive the right treatment as soon as possible.
We are aware that some patients are experiencing difficulties logging in to complete their form. This may be because we do not have the correct address on file for you.
If this is the case, please do not worry, call your hospital's switchboard and a member of our hospital team will help you.
If you were able to complete the form, thank you. The information you have provided has been recorded and you do not need to do anything more.
Contact us if you need to:

We are experiencing delays in answering the switchboard phone due to Covid-19 pressures. We answer emergency calls and external calls from the public as a priority. We apologise for any inconvenience caused.
Please visit our dedicated hospital pages.
We provide services from many other locations.
If you are not able to keep your appointment, or need further details, please tell us as soon as possible. Call the telephone number on your appointment letter or card. Your slot will then transfer to another patient.
You may be unhappy with any aspect of care provided for either you, your relative, or friend. Please raise the issue with us as soon as possible so that we can address it. Find out how to tell us what you think.
You have the right to a copy of information we hold about you. View information and guidance on your own health records. Do not make requests for personal information under the Freedom of Information Act.

Our Patient Advice and Liaison Service (PALS) team gives free and confidential support, information, and advice. This is available to all patients and their relatives, friends, and carers.
