This unique model strengthens primary care by combining direct patient treatment with outreach to professionals, schools, and local communities.
Focusing on high-risk and diverse populations, the team improves heart health through clinics, GP outreach, and prevention-focused education.

Care for complex blood pressure and cholesterol conditions
We accept GP and hospital referrals for patients with difficult-to-manage hypertension or lipid conditions, with clinics at St Bartholomew’s and Royal London Hospitals. When appropriate, we also support GPs to manage care locally, and hospital teams can refer patients directly after a heart attack.
For clinicians
We have CVD Prevention Nurses working in some Primary Care GP Practices in Tower Hamlets. They are a great first point of call to determine what patients can remain under primary care with their assistance, and which patients should be referred to our Consultant clinics.
For difficult-to-manage hypertension, familial hypercholesterolaemia and other challenging lipid diseases, please refer to our Consultant Clinics. For GPs: referrals are only accepted by ERS after screening through Advice and Guidance:
- Hypertension: search for complex blood pressure
- Lipids: search for lipids
Other health professionals should have patients seek access via their GP.

Prevention, education, and community outreach
We provide preventative heart health checks and treatment through hospital clinics and GP-based nursing services across East London. Through ELOPE, we also deliver heart health education, CPR training, and screening in schools, workplaces, and community groups.
